As Canberra's population ages, many older residents face challenges navigating the aged care system, particularly those without family or close supports. The Care Finder program, delivered locally by Community Options, offers free, personalized support to help these individuals access aged care services and community supports.
Care Finder assists with tasks such as registering with My Aged Care, preparing for or attending assessments, completing forms, and connecting people with appropriate local services. The program is designed for those who have no trusted family or friends to help, or who face barriers like language challenges, digital exclusion, social isolation, or reluctance to engage with government systems.
Community Options, an independent not-for-profit organization based in Canberra, brings decades of local experience to this work. The organization tailors support to each individual's unique situation, focusing on building trust and empowering people. This approach aims to strengthen community resilience and inclusion.
The service is funded by the ACT PHN through the Australian Government's PHN Program. For many older Canberrans, Care Finder serves as a bridge to independence, dignity, and belonging, making a tangible difference in their aged care journey.
